Used to record personal Cadet information necessary for TEXAS Div/CHIEF TS
5. NSCADM 004 - PARENTAL AGREEMENT
Used to request assistance from Cadet parents with unit operations and support.
6. NSC-20 - REPORT OF MEDICAL HISTORY FORM
Used to obtain personal medical history on NSCC/NLCC Cadets.
7. NSC-21 - REPORT OF MEDICAL EXAMINATION FORM
Used to record personal medical exam information on NSCC/NLCC Cadets.

OFFICERS, ADULT LEADERS AND MIDSHIPMEN - ENROLLMENT FORMS

Please note that all adult leaders will be subject to a full background check.

1. NSCADM 003 - OFFICER/INSTRUCTOR/MIDSHIPMAN APPLICATION
Used to record pertinent personal Officer/Instructor/Midshipman information
as well as standard releases and privacy statement.
2. NSCADM 005 - OFFICER VOLUNTEER QUESTIONNAIRE
Used to obtain personal references of potential adult volunteers.
3. NSC-20 - REPORT OF MEDICAL HISTORY FORM
Used to obtain personal medical history on NSCC/NLCC Personnel.
4. NSC-21 - REPORT OF MEDICAL EXAMINATION FORM
Used to record personal medical exam information on NSCC/NLCC Personnel.
